Dear Families,
 
Every year the NYC Department of Education asks families to take the NYC School Survey. This survey is designed to collect feedback from families regarding their child’s school and education. School leaders use these survey results to reflect on and improve their schools and programs.  Today, your child will be coming home with an envelope containing information about the school survey. Please keep an eye out for an envelope with a green band on the side.
 
The survey is confidential, and no one from PS 89 will ever see your individual responses.  We encourage all of you to take this opportunity to provide feedback to PS 89.  We hope to have 100% participation.  Every child enrolled at PS 89 as of October 2024 will receive a survey envelope with directions.  So if you have two children enrolled, for example, you will have two surveys to complete.  

You can easily complete the survey online in two ways:
 
1.  The EASIEST way is to log into your NYC Schools Account (NYCSA).  You can click on the School Survey link in the banner at the top of the screen, OR click on the button that says “Forms.” 

                                    


You will see a “Take Survey” button, which will direct you to the survey questions.
 
                                                


2.  You can also complete your survey by going to the NYC School Survey website. You will need to submit a code which is in your child's school survey envelope.  Your child’s OSIS # is the 9-digit number underneath your child's name on the paper survey.



Lastly, you still have the option to mail in the paper survey enclosed in your envelope.  The NYC School Survey window closes on Friday, April 4th, but don't wait until then to complete your survey!  We thank you in advance for taking the time to share your thoughts!
